Regional Issues Log
A state-wide Issues Management Pathway has been introduced to capture systemic issues associated with National Aged Care reforms, including the Commonwealth Home Support Program and Home and Community Care Program for Younger People transitions. This strategy includes the recording of regional issues and monthly submission of an issues log to both the Department of Health and the Department of Health and Human Services.
The Issues Log is designed to:
identify and manage issues raised by regional service providers
refine the recording and reporting of regional issues to DoH and DHHS by using a streamlined
and consistent approach across Victoria
cover systemic service providers issues relating to the CHSP, HACC-PYP, sector development
and change management
promote discussion and design of strategies to identify emerging systemic issues.
This strategy is not intended to replace existing issues management approaches such as for My Aged Care issues resolution processes.
Each of the nine Victorian Regional Development Coordinators submit a monthly log to DHHS and DoH, reporting any regional issue that appears to be systemic in nature or where clarification is needed.
Feedback may be provided directly to the RDCs for dissemination or through the distribution of program bulletins, Fact Sheets or FAQs, particularly where there is common misunderstanding throughout the sector.
